Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care and assist with activities of daily living
Administer medications and treatments accurately and safely
Monitor and record vital signs, reporting changes to the care team
Document patient status and care delivery in the health record promptly
Collaborate with nurses and interdisciplinary team members to support care plans
Support resident comfort and dignity while promoting independence
Follow infection control and safety protocols to protect patients and staff
Participate in shift handoffs and communicate critical information clearly

Qualifications

Active Licensed Practical Nurse (LPN) license (Pennsylvania license expected for this location)
Current BLS/CPR certification or willingness to obtain upon hire
Proof of required immunizations and health screenings per facility policy
Strong communication and teamwork skills with a patient-centered mindset
Ability to perform physical tasks typical of nursing care, including assisting patients with mobility

Education

Completion of an accredited Licensed Practical Nurse program
High school diploma or GED

Knowledge & Skills

Medication administration and basic wound care
Patient assessment and vital sign monitoring
Accurate clinical documentation and charting
Infection prevention and standard precautions
Prioritization and time management in a clinical setting
Compassionate bedside manner and effective communication
