Roles & Responsibilities

Provide compassionate, patient-centered nursing care to residents
Administer medications and treatments safely and accurately
Document patient status and interventions clearly in the chart
Collaborate with RNs, physicians, and interdisciplinary team to optimize care
Monitor patients for changes and escalate concerns promptly
Support patient activities of daily living while promoting independence

Qualifications

Current LPN/LVN license (Pennsylvania license preferred)
Basic Life Support (BLS/CPR) certification or ability to obtain upon hire
Strong communication skills and ability to work in a team
Ability to pass background checks and required health screenings
Compassionate approach to patient care and reliable attendance

Education

LPN/LVN diploma or certificate from an accredited nursing program

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and infection control procedures
Familiarity with electronic health records and charting
Good clinical assessment and vital signs monitoring
Time management and prioritization in a fast-paced setting
