Roles & Responsibilities

• Provide compassionate direct nursing care to residents and ensure their comfort
• Administer medications and treatments accurately and on schedule
• Monitor and document vital signs, clinical changes, and resident progress
• Collaborate with the interdisciplinary care team to implement individualized care plans
• Assist residents with activities of daily living while promoting dignity and independence
• Maintain clear, timely, and accurate electronic charting and documentation
• Educate residents and families about care plans, medications, and health needs
• Follow infection control, safety protocols, and facility policies to maintain a safe environment

Qualifications

• Current LPN license in Connecticut (CT) or eligibility to obtain licensure
• Graduate of an accredited Practical Nursing program
• Current BLS/CPR certification or willingness to obtain upon hire
• Strong communication and interpersonal skills with residents and families
• Ability to work effectively as part of a team in a fast-paced care setting
• Previous experience in long-term care or skilled nursing is a plus

Education

• Graduate of an accredited Practical Nursing (LPN) program
• Current Connecticut LPN license or eligibility to obtain

Knowledge & Skills

• Medication administration and documentation best practices
• Monitoring and interpreting vital signs and clinical status
• Basic wound care and dressing management
• Electronic medical record (EMR) charting proficiency or willingness to learn
• Infection prevention and control procedures
• Resident-centered care and effective family communication
• Time management and prioritization in a clinical environment
