Roles & Responsibilities

Provide compassionate, direct patient care consistent with LPN scope of practice
Administer medications and treatments accurately and document thoroughly
Monitor and report changes in patient condition to the nursing team
Maintain clear, timely documentation in the patient record
Collaborate with nurses, CNAs, and interdisciplinary staff to deliver coordinated care
Support residents and families with respectful communication and care education

Qualifications

Current Licensed Practical Nurse (LPN) credential (role requires LPN skills — core skill flagged in the posting)
Ability to work 8-hour shifts as scheduled (posting lists total hours: 8)
Active eligibility or authorization to work in Pennsylvania (facility located in Lancaster, PA)
Professional attitude, reliable attendance, and strong teamwork skills

Knowledge & Skills

Solid working knowledge of basic nursing assessments and care planning
Medication administration and safe documentation practices
Clear communication skills with patients, families, and clinical teams
Comfortable using electronic health records and standard clinical forms
Ability to prioritize tasks in a busy long-term care environment
