Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, direct patient care aligned with facility care plans
Administer medications and treatments accurately and document thoroughly
Monitor patients' conditions and report changes to the interdisciplinary team
Support activities of daily living and promote patient dignity and comfort
Collaborate with nurses and allied staff to ensure seamless care transitions
Maintain accurate, timely charting and follow regulatory documentation standards
Contribute to a positive resident experience through respectful communication

Qualifications

Valid LPN license (PA preferred) — current and in good standing
Current BLS/CPR certification
Previous experience in long term care, skilled nursing, or similar settings preferred
Strong communication and interpersonal skills with residents and families
Ability to pass background and health screenings per facility policy
Dependable, team-oriented approach with strong attention to detail

Education

Graduate of an accredited LPN program
High school diploma or GED

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and ADL assistance
Familiarity with electronic charting systems or willingness to learn
Understanding of infection control and safety procedures
Time management and prioritization in a fast-paced care environment
Empathy-driven resident care and excellent bedside manner
