Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, high-quality direct patient care and promote resident dignity
Administer medications and treatments accurately and in a timely manner
Perform focused patient assessments and report changes to the care team
Document care thoroughly and maintain accurate patient records in the EHR
Collaborate with nurses, therapists, and interdisciplinary staff to support care plans
Assist with admission, transfer, and discharge processes to ensure smooth transitions
Adhere to infection control, safety protocols, and regulatory requirements

Qualifications

Active LPN license in the state of Pennsylvania (required)
Current BLS/CPR certification (required or obtain upon hire)
Completion of an accredited LPN program (diploma or certificate)
Previous practical nursing experience preferred; new grads are welcome to apply
Strong communication skills and the ability to work well in a team-based setting
Ability to pass background checks and meet facility onboarding requirements

Education

LPN diploma or certificate from an accredited program
High school diploma or GED

Knowledge & Skills

Medication administration and documentation best practices
Basic patient assessment and vital sign monitoring
Familiarity with infection prevention and safe patient handling
Comfortable using electronic health records and clinical documentation
Good time management and prioritization in a fast-paced environment
Empathetic bedside manner and strong interpersonal skills
