Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients with dignity
• Administer medications and treatments accurately and document thoroughly
• Monitor patient conditions, report changes, and escalate concerns to the RN or provider
• Support care plans by collaborating with interdisciplinary team members
• Perform skilled nursing tasks such as wound care, vital signs, and ADL assistance
• Maintain accurate electronic and paper charting in accordance with facility policies
• Contribute to a safe, clean environment by following infection control practices
• Support patients and families with education, reassurance, and clear communication

Qualifications

• Current LPN (Licensed Practical Nurse) license — Pennsylvania licensure recommended
• Completion of an accredited LPN/Vocational Nursing program
• Basic Life Support (BLS) certification (or willingness to obtain)
• Current immunizations and ability to pass background and health screenings
• Professional, patient-focused attitude with strong teamwork and communication skills
• Previous LPN experience in a clinical or long-term care setting preferred

Education

• Graduate of an accredited LPN/Vocational Nursing program (High school diploma or GED required)

Knowledge & Skills

• Strong clinical assessment and patient observation skills
• Medication administration and documentation accuracy
• Experienced with basic wound care and ADL support
• Comfortable using electronic health records and charting systems
• Solid infection control practices and safety awareness
• Excellent interpersonal skills for patient and family interactions
