Roles & Responsibilities

• Provide compassionate, competent nursing care to residents according to care plans
• Administer medications and treatments safely and accurately
• Monitor and document patient conditions, reporting changes promptly to RN/physician
• Collaborate with the interdisciplinary team to support resident-centered care
• Assist with activities of daily living and promote patient dignity and independence
• Participate in shift handoffs and maintain clear, timely charting in the medical record

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania or eligibility to obtain one
• Current Basic Life Support (BLS) certification (or willingness to obtain)
• Comfortable administering medications and performing routine nursing procedures
• Strong communication and teamwork skills; reliable and patient-centered
• Previous experience in long-term care or similar settings preferred but not required

Education

• Graduate of an accredited Practical Nursing/Vocational Nursing program
• Completion of state LPN licensure requirements

Knowledge & Skills

• Medication administration and documentation
• Basic patient assessment and vital signs monitoring
• Infection control and standard precautions
• Charting and electronic medical record familiarity
• Effective communication with patients, families, and care teams
• Time management and the ability to prioritize clinical tasks
