Roles & Responsibilities

Provide compassionate hands-on nursing care to residents and patients with respect and dignity
Administer medications and treatments accurately and timely according to plan of care
Perform routine assessments and document changes in condition clearly and promptly
Collaborate with the interdisciplinary team to create and follow individualized care plans
Assist with ADLs (activities of daily living) while promoting independence whenever possible
Communicate effectively with residents, families, and team members to support positive outcomes
Participate in quality improvement efforts and follow facility policies and procedures

Qualifications

Active LPN license in Pennsylvania (required)
Graduation from an accredited LPN/Vocational Nursing program
Current BLS/CPR certification (or willingness to obtain)
Ability to pass background and health screenings as required by facility
Strong communication skills and a patient-centered approach
Previous long-term care or med-surg experience preferred but not required

Education

Graduate of an accredited LPN/Vocational Nursing program
High school diploma or equivalent

Knowledge & Skills

Medication administration and basic IV/shot familiarity
Wound care basics and dressing changes
Patient assessment, vital signs, and recognizing changes in condition
Clear, timely clinical documentation and charting
Comfortable collaborating within a multi-disciplinary care team
Basic electronic medical records (EMR) proficiency
