Roles & Responsibilities

Provide compassionate, direct patient care including assistance with activities of daily living
Take and record vital signs accurately and report changes to nursing staff
Assist with safe transfers, mobility, and ambulation to maintain patient safety
Maintain a clean, comfortable environment and follow infection-control practices
Document patient interactions and interventions clearly and promptly
Support the interdisciplinary team to promote quality patient outcomes
Build trusting relationships with residents and their families through clear communication

Qualifications

Current CNA certification (required) — Pennsylvania preferred given the facility location
CPR/BLS certification preferred or willing to obtain
Ability to pass background checks and required health screenings
Comfortable with patient lifting/moving techniques and safe body mechanics
Strong communication skills and a collaborative attitude

Education

High school diploma or GED preferred
Completed CNA training program and current certification

Knowledge & Skills

Solid knowledge of basic patient care and ADLs (bathing, dressing, feeding)
Ability to take accurate vital signs and observe patient condition
Familiarity with infection control and safety protocols
Competence in documentation and following care plans
Good interpersonal skills for working with residents, families, and staff
