Roles & Responsibilities

• Provide compassionate personal care to residents, assisting with activities of daily living
• Take and record vital signs and report changes in patient condition to nursing staff
• Assist with safe transfers and mobility, following facility protocols
• Maintain accurate and timely documentation of care and observations
• Support infection control and safety practices to protect residents and team members
• Communicate clearly and respectfully with residents, families, and the interdisciplinary team
• Help create a warm, dignified environment that promotes resident comfort and independence

Qualifications

• Active CNA certification in the state of Pennsylvania (required)
• Current BLS/CPR certification preferred or willingness to obtain
• Completed background screen and up-to-date immunizations per facility policy
• Strong communication and interpersonal skills, with a resident-first mindset
• Ability to perform physical aspects of the role, including assisted transfers
• Prior experience in long-term care or a similar healthcare setting is a plus

Education

• High school diploma or equivalent
• Completed CNA training program and active CNA certification

Knowledge & Skills

• Knowledge of basic nursing care and activities of daily living
• Familiarity with infection prevention and safety procedures
• Comfortable with documentation and basic charting practices
• Ability to work effectively as part of a multidisciplinary team
• Time management and prioritization skills in a busy care environment
• Compassionate bedside manner and strong observational skills
