Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in a long-term care environment
- Administer medications and treatments accurately and document per facility policy
- Monitor resident status, record vital signs, and report changes to the care team
- Collaborate with RNs, CNAs, therapists, and physicians to deliver coordinated care
- Assist with admissions, transfers, and discharge planning when needed
- Support rehabilitation goals and daily living activities to maximize resident dignity
- Maintain accurate clinical documentation in the resident chart and electronic records
- Follow infection control standards and facility policies to ensure a safe environment
Qualifications
- Active LPN license in New York (required)
- Completion of an accredited practical nursing program
- Current BLS/CPR certification (or willing to obtain prior to start)
- Ability to pass background and health screenings per facility requirements
- Prior experience in long-term care or skilled nursing preferred but new grads considered
- Strong communication, teamwork, and resident-focused bedside manner
Education
- Practical Nursing Certificate or Diploma from an accredited program; high school diploma or GED required
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and skin integrity awareness
- Proficient in taking and interpreting vital signs
- Familiarity with electronic health record (EHR) charting or willingness to learn
- Good clinical judgment and the ability to escalate concerns appropriately
- Excellent interpersonal skills for working with residents and families
