Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in a skilled nursing and long term care setting
- Assess resident conditions, develop and update care plans, and document clinical findings accurately
- Administer medications and treatments safely and according to policy
- Monitor and report changes in resident status to the interdisciplinary team
- Support activities of daily living while promoting dignity and independence
- Collaborate with physicians, therapists, and families to coordinate care
- Participate in infection control, safety practices, and quality improvement initiatives
Qualifications
- Active Registered Nurse (RN) license in good standing for the state of practice
- Experience or interest in Skilled Nursing Facility (SNF) and Long Term Care (LTC) settings
- Up to date immunizations and health requirements: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine, current physical/health statement
- Successful completion of Trinity 10 Panel Drug Screen and background checks as required
- Current BLS certification; ACLS or other certifications a plus
- Strong communication skills and ability to work independently in a fast paced environment
Education
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
- Active RN license required
Knowledge & Skills
- Thorough knowledge of long term care nursing practices and regulations
- Medication administration, wound care, and basic clinical procedures
- Skilled assessment, critical thinking, and accurate documentation
- Familiarity with electronic medical records and care planning
- Infection prevention and control best practices
- Ability to collaborate effectively with interdisciplinary teams and families
