Roles & Responsibilities

Conduct thorough psychosocial assessments to understand patients' strengths, needs, and risk factors
Develop individualized, evidence-based treatment plans that support recovery and improve functioning
Deliver therapeutic interventions across the lifespan for depression, anxiety, trauma, substance use, and adjustment disorders
Collaborate closely with primary care providers, nurses, and behavioral health colleagues to coordinate whole-person care
Maintain timely, accurate clinical documentation and treatment summaries that support continuity of care
Coordinate referrals and community resources to support patients in rural and medically underserved settings
Advocate for patients and help remove barriers to care while promoting resilience and wellness

Qualifications

Master's degree in Social Work from an accredited program (required)
Active LISW license in Iowa (required)
Minimum of 2 years of clinical experience preferred
Experience in rural or medically underserved settings is a plus
Strong interpersonal, organizational, and clinical documentation skills
Commitment to evidence-based practice and collaborative, integrated care

Education

Master's degree in Social Work (MSW) from an accredited program

Knowledge & Skills

Psychosocial assessment and diagnostic formulation
Treatment planning and evidence-based therapeutic interventions
Behavioral health practices across the lifespan
Clinical documentation and care coordination
Trauma-informed care and substance use treatment approaches
Collaborative work with primary care teams and interdisciplinary communication
Strong organizational and interpersonal skills for rural practice
