Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, patient-centered nursing care with professionalism and warmth
Administer medications and treatments according to care plans and facility policies
Perform timely and thorough patient assessments, documenting findings in the EHR
Collaborate with RNs, physicians, and interdisciplinary team members to optimize patient outcomes
Educate patients and families about care plans, medications, and discharge instructions
Follow infection control and safety protocols to maintain a safe care environment
Support admission, transfer, and discharge processes to ensure smooth patient transitions

Qualifications

Active LPN license in Pennsylvania (required)
Completion of an accredited LPN/vocational nursing program
Basic Life Support (BLS) / CPR certification preferred or able to obtain
Strong communication and interpersonal skills with a patient-first mindset
Comfortable with electronic health record documentation
Previous experience in long-term care or related setting is a plus

Education

LPN diploma or vocational nursing certificate from an accredited program

Knowledge & Skills

Medication administration and documentation best practices
Routine patient assessments, vital signs, and basic clinical procedures
Wound care basics and skin integrity monitoring
Familiarity with EHR systems and accurate charting
Infection prevention and safety protocols
Team collaboration and effective handoff communication
