Roles & Responsibilities

Provide safe, patient-centered nursing care in the immediate care clinic following established standards and within LPN scope of practice.
Take and interpret vital signs and perform initial patient assessments to support timely, appropriate care.
Triage patient calls, prioritize care needs, and escalate to providers or RN/Clinical Managers when required.
Manage medication refills and efficiently complete prior authorization requests to keep patients on their therapy.
Act as the primary liaison between patients, clinical staff, and providers to ensure smooth, coordinated care.
Give direction to CNAs and Schedulers and collaborate with interdisciplinary teammates to meet patient needs.
Maintain current licensure and Basic Life Support (BLS) certification, and follow state/federal continuing education requirements.

Qualifications

Current State LPN license required
Graduate of an accredited nursing program (LPN)
Current Basic Life Support (BLS) certification
Demonstrated ability to take vital signs, triage patient calls, and perform patient assessments
Experience handling medication refills and prior authorization processing
Comfort providing direction to CNAs and coordinating with schedulers and providers
Willingness to work a primarily M-F schedule with occasional weekends as needed

Education

Graduate of an accredited Nursing Program (LPN)
Maintain continuing education as required by state/federal licensure

Medication management and refill workflows
Prior authorization processing
Basic Life Support (BLS)
Supervising CNAs and coordinating with schedulers
Interdisciplinary team collaboration and patient-centered communication
