Contract Type: Per-Diem

Shift Type: 8-hour shift (total hours: 8). Specific start/end times and days to be confirmed with hiring manager.

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate daily nursing care to residents, focusing on safety and dignity
Administer medications and treatments accurately and document per facility policy
Monitor resident condition and promptly report changes to the care team
Assist with care plans and collaborate with RNs and interdisciplinary staff to achieve positive outcomes
Support residents with activities of daily living and promote independence where possible
Communicate clearly and kindly with residents and families to build trust and understanding
Maintain accurate, timely charting and comply with facility standards and regulations

Qualifications

Active Licensed Practical Nurse (LPN) license in Pennsylvania
Current Basic Life Support (BLS/CPR) certification preferred or obtained upon hire
Strong clinical assessment skills and attention to detail
Excellent communication and teamwork skills
Comfort working in long term care environments and with older adult populations

Education

Completion of an accredited LPN/Vocational Nursing program or equivalent

Knowledge & Skills

Medication administration and documentation best practices
Familiarity with long term care nursing routines and resident-centered care
Basic wound care, vital signs monitoring, and ADL support
Ability to use electronic medical records and follow policy-driven workflows
Good time management and prioritization in a busy care setting
