Roles & Responsibilities

• Evaluate residents and develop individualized treatment plans that restore independence
• Provide therapeutic interventions to improve function, ADLs, and safety
• Collaborate closely with nursing and rehab staff to ensure coordinated care and great outcomes
• Maintain timely, accurate documentation in compliance with state and federal regulations
• Use clinical judgement to progress plans of care and provide resident-centered recommendations
• Participate in team meetings, care planning, and family education to support continuity of care

Qualifications

• Current California Occupational Therapist (OT) license required
• Strong interpersonal and clinical skills with a commitment to quality care
• Experience in skilled nursing, post-acute, or long-term care preferred
• Comfortable with clinical documentation and regulatory compliance
• Demonstrated ability to collaborate within an interdisciplinary team
• Willingness to engage in mentorship, continuing education, and professional growth

Knowledge & Skills

• Proficient in evaluation and treatment planning for adult/geriatric populations
• Skilled in therapeutic interventions to address ADLs, mobility, and safety
• Solid clinical documentation practices and knowledge of regulatory requirements
• Ability to work effectively with nursing and rehabilitation teams
• Strong communication, problem solving, and patient-centered care approaches
• Familiarity with modern therapy gym equipment and adaptive devices
