Roles & Responsibilities

• Provide compassionate bedside and in-home care to home health and hospice patients
• Administer medications and treatments according to plan of care and scope of practice
• Assess patient condition, monitor symptoms, and report changes to the care team
• Coordinate care with nurses, providers, and family caregivers to support holistic needs
• Document assessments, interventions and patient responses accurately and timely
• Educate patients and families on symptom management, medication use and safety
• Support end-of-life comfort measures and communicate respectfully with patients and families
• Participate in required competency testing and maintain current certifications

Qualifications

• Active LPN license (must be valid in Michigan or appropriate jurisdiction)
• BLS certification required
• Experience or interest in home health and hospice patient care
• Successful completion of Trinity Specialty Specific Competency Testing as required
• Respirator fit test and Trinity respirator evaluation completion where applicable
• Required health screening and immunizations: TB testing (2-step PPD or IGRA), MMR, Hepatitis B, Varicella, Tdap, Influenza, COVID-19 per facility policy
• Trinity 10 panel drug screen and license/certificate verification required
• Ability to travel to patient homes and work independently with strong communication skills

• Medication administration and basic wound care
• Infection control and immunization protocols
• Comfort with electronic documentation and timely charting
• Strong interpersonal skills for family-centered care and care coordination
• Ability to follow Trinity-specific competency checklists and testing procedures
