Roles & Responsibilities

• Provide compassionate, patient-centered care to older adults in a geriatric setting
• Follow facility policies and complete Trinity Specialty Specific Competency Testing as required
• Maintain accurate documentation and verify required licenses and certifications
• Participate in mandatory safety and infection control practices, including Fit Test completion
• Collaborate with the interdisciplinary team to support patient care plans and transitions
• Assist in assessments and interventions that promote dignity, mobility, and quality of life

Qualifications

• BLS certification (required)
• License/Certificate verification completed prior to start (as applicable to role)
• Successful completion of Trinity Specialty Specific Competency Testing
• Fit Test on file (required)
• Ability to work per-diem/open schedule and adapt to varying shift needs
• Current, valid professional licensure if applicable to the SPHP role

Knowledge & Skills

• Understanding of geriatric care principles and common age-related conditions
• Strong communication and teamwork skills to coordinate with interdisciplinary staff
• Attention to detail for documentation and compliance tasks
• Comfort with facility protocols for testing, fit testing, and competency evaluations
• Adaptability and ability to work independently in per-diem assignments
