Roles & Responsibilities

Evaluate patients with speech, language, cognitive, and swallowing disorders to build a clear clinical picture
Develop and implement individualized treatment plans that target meaningful functional gains
Provide skilled therapy services focused on communication, cognition, and dysphagia management
Document patient progress and treatment outcomes accurately and professionally
Collaborate with interdisciplinary healthcare teams to support optimal patient outcomes
Educate patients and caregivers on therapy goals, home strategies, and safety to maximize carryover

Qualifications

Active California Speech-Language Pathologist license (required)
Experience in a Skilled Nursing Facility or rehabilitation setting preferred
Strong clinical judgment with excellent communication skills
Ability to work independently while contributing positively to a team
Comfort with skilled documentation and outcome-driven care

Knowledge & Skills

Assessment and treatment of communication disorders
Dysphagia evaluation and management (swallowing disorders)
Cognitive-communication and language disorder interventions
Skilled documentation and tracking of treatment outcomes
Interdisciplinary collaboration and patient/caregiver education
Familiarity with rehab-focused SNF workflows and regulatory expectations
