Roles & Responsibilities

• Assess patients with communication, cognitive, and swallowing disorders using standardized assessments
• Develop and implement individualized treatment plans based on evidence-based practice
• Evaluate progress, adjust interventions, and document outcomes in the EMR
• Provide patient, family, and caregiver education and counseling to support recovery
• Prepare comprehensive discharge summaries that meet regulatory requirements
• Collaborate with interdisciplinary team members about barriers, goals, and treatment plans
• Accurately record treatment minutes and maintain clinical documentation per policy
• Obtain necessary DME or supplies to support patient care where appropriate
• Participate in care conferences, utilization review, and facility trainings
• Supervise and mentor CFY or student SLPs to support the next generation of clinicians

Qualifications

• Active ASHA CCC (required)
• Missouri state SLP license (required); compact allowed
• Minimum 1 year of SLP experience (new grads accepted with Level II clinical experience)
• Long-Term Care (LTC) or rehab experience (required)
• Comfortable using electronic documentation/EMR systems
• Ability to supervise CFY or student clinicians
• Willingness to participate in facility meetings and required trainings
• CCS credential helpful (3 years holding active CCS preferred)
• Able to provide one professional reference at time of submission

Knowledge & Skills

• Proficiency with standardized assessments and outcome measurement
• Strong documentation skills and familiarity with EMR workflows
• Evidence-based clinical decision making for evaluation and treatment
• Knowledge of Medicare, Managed Care, and productivity expectations in LTC/rehab settings
• Experience with group therapy and strategies to manage caseload productivity
• Good interdisciplinary communication and care coordination abilities
• Ability to identify and procure appropriate DME and supplies
