Location: Prescott, Arizona

End Date: 09-28-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM, weekends every 3-4 weeks if at all

Hours per Week: 40

Candidate Pay Rate: $50 per hour

Roles & Responsibilities

Complete comprehensive Social Work assessments for patients across multiple units
Conduct Mental Health assessments and support patients in crisis
Coordinate discharge planning to ensure smooth transitions of care
Assist with Title 36 petitions and connect patients with vital resources
Address homelessness, substance abuse, and social determinants of health
Provide maternal child assessments and support families in need
Report to DCS and APS as needed to protect vulnerable populations

Qualifications

Active LMSW or LCSW license in Arizona
Experience with Cerner EMR preferred
Strong assessment and care coordination skills
Compassionate approach to supporting diverse patient needs

Knowledge & Skills

Social Work assessments and Mental Health evaluations
Discharge planning and care coordination
Understanding of SDOH, homelessness, and substance abuse resources
Title 36 petition processes
DCS and APS reporting requirements
Maternal child health assessments
