Roles & Responsibilities

Conduct OASIS assessments and Start of Care evaluations
Provide personalized physical therapy to patients recovering from falls and fractures
Travel across four rural counties delivering compassionate care
Document visits accurately using tablet technology
Build meaningful relationships with elderly patients and their families

Qualifications

Current Physical Therapist license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable working independently in rural settings
Strong communication and time management skills
