Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits for orthopedic and geriatric patients when nursing is not ordered
Help patients regain mobility and independence after joint replacements and managing chronic conditions like CHF and COPD
Complete approximately five meaningful patient visits each day using a company-provided tablet
Collaborate with an intake RN and manager to coordinate care and ensure excellent outcomes

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than two years of experience if they have Home Health background
Comfortable traveling within the service area and managing a daily mileage of approximately 40 miles

Knowledge & Skills

HomeCare HomeBase EMR experience is a plus
Strong assessment and care-planning skills for orthopedic and medically complex patients
Ability to work independently while staying connected with the care team
Excellent communication and documentation abilities
