Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Help patients recover from surgeries, falls, and other conditions with personalized care plans
Travel within a 60-mile radius to deliver therapy where patients need it most
Collaborate with interdisciplinary teams to ensure comprehensive patient outcomes
Document care using HomeCare/HomeBase EMR system

Qualifications

Current ND Physical Therapy License required
BLS certification needed
1+ year of previous home health experience
Valid driver's license and auto insurance
HomeCare/HomeBase EMR experience preferred

Knowledge & Skills

Physical therapy expertise in home health settings
Strong communication and patient education skills
Ability to work independently in diverse home environments
Comfortable managing 5-7 patients per day
Familiarity with common diagnoses like post-surgical recovery and fall prevention
