Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct start-of-care assessments, recertifications, and resumption-of-care visits
Deliver 5 to 6 meaningful patient visits each day, adjusting for admissions or travel
Collaborate with a supportive clinical team available for guidance in the field
Complete OASIS documentation and ensure high-quality care coordination
Perform occasional safety evaluations or teaching visits in hospice situations

Qualifications

Current ND Physical Therapy License
BLS certification
Valid driver's license and auto insurance
Home Health experience and OASIS training preferred
First-time travelers welcome

Strong communication and time-management abilities
Comfort working independently in patient homes
