Roles & Responsibilities

• Conduct thorough patient evaluations to determine individualized therapy needs
• Provide daily treatments that help residents regain function and confidence
• Manage timely and safe discharges, coordinating with the care team and families
• Maintain accurate, compliant therapy documentation for every patient
• Collaborate closely with the facility’s therapy team to deliver cohesive care
• Meet productivity expectations and follow CRG policies and procedures

Qualifications

• SLP clinical background (Speech-Language Pathology) — this role is for an SLP
• 1-3 years of clinical experience (as listed in the job requirements)
• Prior Skilled Nursing Facility (SNF) experience is strongly preferred
• Ability to be present on-site during scheduled hours (on-site presence required for guaranteed hours)
• Willingness to follow facility dress code (black scrubs) and CRG policies
• Comfort with productivity tracking and meeting documented productivity requirements (90%)

Knowledge & Skills

• Skilled in patient evaluations and creating individualized treatment plans
• Experienced with daily treatment delivery in a SNF environment
• Strong therapy documentation skills and compliance awareness
• Familiarity with productivity tracking and meeting performance metrics
• Good interpersonal skills for team collaboration and patient/family communication
