Roles & Responsibilities

• Provide compassionate, skilled nursing care to long-term care patients in a home health setting as a member of the Per Diem pool
• Perform thorough patient assessments and develop/implement individualized care plans
• Administer medications and treatments safely while documenting care accurately
• Collaborate with interdisciplinary teams and communicate with patients and families to support continuity of care
• Complete Trinity specialty competency testing and other required competency evaluations
• Adhere to infection control procedures, participate in respirator evaluations and fit testing, and maintain required health screenings

Qualifications

• Active RN license (license/certificate verification required)
• BLS certification required
• Completion of Trinity specialty specific competency testing as part of onboarding
• Ability to complete required health screenings and immunizations: 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, influenza vaccine
• Successful completion of Trinity 10-panel drug screen, respirator evaluation, fit test, color vision check, and physical/health statement

Knowledge & Skills

• Experience or familiarity with long term care and home health nursing practices
• Strong patient assessment and medication administration skills
• Accurate clinical documentation and comfort with electronic health records
• Infection prevention and control knowledge
• Good communication, teamwork, and time-management abilities
• Willingness to complete specialty competency testing and follow established protocols
