Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to long-term care participants
• Perform thorough assessments and develop individualized care plans
• Administer medications and treatments safely and accurately
• Collaborate with interdisciplinary team members to coordinate care
• Document clinical findings and interventions clearly and promptly
• Participate in skills validation and competency checks (Trinity Skills Checklist)
• Support infection prevention practices and maintain a safe care environment

Qualifications

• Active Registered Nurse (RN) license (license/certificate verification required)
• BLS certification required
• Experience or interest in Long-Term Care (LTC) settings
• Successful completion of Trinity Skills Checklist and required onboarding
• Required health clearances and screenings (Fit Test, 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine, Trinity 10-panel drug screen)
• Ability to work flexible per-diem shifts and adapt to changing unit needs

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Familiarity with long-term care best practices and resident-centered care
• Competency with documentation and electronic health records
• Infection control and safe work practices
• Effective communication and teamwork in interdisciplinary settings
• Comfort completing skills checklists and competency-based evaluations
