Roles & Responsibilities

• Provide compassionate, skilled nursing care to long-term care participants, focusing on safety and dignity
• Perform thorough assessments and monitor changes in condition, intervening as needed
• Administer medications and treatments accurately and document in the medical record
• Develop and update individualized care plans in collaboration with the care team
• Educate participants and families about health conditions, treatments, and preventive care
• Communicate clearly with interdisciplinary partners to ensure coordinated, high-quality care
• Maintain regulatory and documentation standards, including timely charting and reporting

Qualifications

• Active RN licensure (Registered Nurse) — ability to practice in Louisiana is preferred
• Experience or interest in long-term care, geriatric nursing, or community-based care
• Current BLS (Basic Life Support) certification required or obtainable upon hire
• Required health documentation: MMR, Hepatitis B, Varicella, Tdap, Physical/Health Statement
• Required vaccinations and screens: COVID-19 vaccine, annual influenza vaccine, and Trinity 10 Panel drug screen
• Strong clinical judgment, excellent communication skills, and a patient-centered approach

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) — RN licensure required

Knowledge & Skills

• Skilled in clinical assessment and early recognition of changes in condition
• Proficient with medication administration and care plan implementation
• Comfortable working within interdisciplinary teams and coordinating care
• Good documentation practices and familiarity with electronic health records
• Ability to prioritize tasks, manage time, and handle a varied caseload
• Empathy, patience, and excellent interpersonal skills for working with older adults
