Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care setting
• Assess resident health status, develop and implement individualized care plans
• Administer medications and treatments safely and document care accurately
• Collaborate with interdisciplinary team members to optimize resident outcomes
• Perform and document clinical competency testing and skills checklists as required
• Maintain accurate license and certification records and complete required screenings
• Support infection prevention practices and follow facility policies and procedures

Qualifications

• Active Registered Nurse (RN) license (state licensure required) with license/certificate verification
• BLS certification required
• ACLS certification required
• Experience in long-term care (LTC) preferred or equivalent clinical experience
• Ability to complete Trinity Specialty Specific Competency Testing and Skills Checklist
• Must complete required personal health screenings: 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, physical/health statement, COVID-19 and influenza vaccination as required
• Successful completion of a Trinity 10 panel drug screen

Education

• Graduate of an accredited nursing program (ADN or BSN)
• BSN preferred

Knowledge & Skills

• Strong knowledge of long-term care nursing practices and resident-centered care
• Medication administration and accurate clinical documentation
• Skilled in patient assessment, care planning, and nursing interventions
• Familiarity with infection control, immunization requirements, and screening protocols
• Comfortable with competency testing and following facility-specific skill checklists
• Good communication, teamwork, and time management skills
