Contract Type: Contract

Shift Type: Multiple 8-hour shifts available: 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, weekdays with every other weekend and rotating holidays

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments and develop daily patient lists
Facilitate goals of care discussions and advance care planning
Provide education on disease process, advance directives, and POLSTs
Lead family meetings and provide compassionate end-of-life support
Collaborate with physicians, NPs, and RNs during daily rounding
Coordinate care for 10-15 consults per day across the medical center

Qualifications

Active and unencumbered Registered Nurse license in California
BLS certification required; ELNEC preferred
Minimum of 2-5 years of recent experience in palliative or hospice care
Strong acute care, clinical thinking, and communication skills
Flexibility and organizational skills with a desire to serve others

Knowledge & Skills

Expertise in patient assessment and goals of care discussions
Knowledge of end-of-life care, advance directives, and family dynamics
Proficiency in Cerner EMR and documentation
Excellent interpersonal skills for multidisciplinary collaboration
Ability to work in a faith-based, patient-centered environment
