Roles & Responsibilities

• Provide high-quality skilled nursing care to homebound and hospice patients with compassion and clinical excellence.
• Assess patient condition regularly, develop and update individualized care plans, and document all findings accurately.
• Administer medications and treatments safely while monitoring response and reporting changes to the care team.
• Deliver comfort-focused, palliative and end-of-life care in collaboration with families and interdisciplinary partners.
• Educate patients and caregivers about care plans, symptom management, and available community resources.
• Coordinate with physicians, therapists, social workers, and community agencies to ensure seamless care transitions.
• Maintain infection control standards, complete required screenings, and uphold organizational policies and procedures.
• Participate in competency testing and continuing education to maintain clinical skills and certifications.

Qualifications

• Active RN license (must be verified) — meet state requirements for practice
• Current BLS certification required
• Experience or strong interest in long-term care, home health, and hospice settings preferred
• Willingness to complete Trinity Specialty Specific Competency Testing
• Ability to meet health screening requirements (fit test, 2-step PPD or IGRA, drug screen, immunizations)
• Strong clinical assessment, documentation, and patient/family communication skills

Knowledge & Skills

• Skilled in patient assessment, medication administration, and symptom management
• Comfortable providing palliative and end-of-life care with empathy and clinical expertise
• Proficient documentation and familiarity with electronic medical records (EMR) workflows
• Excellent communication and care-coordination abilities with interdisciplinary teams
• Infection control practices and adherence to clinical safety protocols
• Time management, prioritization, and adaptability in home care environments
• Willingness to complete specialty competency testing (Trinity) and required health screenings
