Roles & Responsibilities

• Provide direct physical therapy interventions under the supervision of a licensed physical therapist
• Implement treatments according to the physician's plan of care and the supervising PT's directions
• Document patient progress clearly and timely in accordance with facility policies
• Collaborate with nursing, physicians, and the interdisciplinary team to optimize patient outcomes
• Educate patients and families on therapeutic techniques, safety, and home exercise programs
• Adhere to the state Physical Therapy Practice Act and facility standards to ensure safe, compliant care

Qualifications

• PTA credential (or completion of an accredited PTA program) and ability to meet state requirements
• Comfortable working in a skilled nursing facility (SNF) environment
• Able to follow a physician's plan of care and work under the direction of a physical therapist
• Open to all experience levels — any experience accepted, including new grads
• Strong communication and documentation skills

Knowledge & Skills

• Direct physical therapy treatment skills and patient-centered care
• Working knowledge of the state Physical Therapy Practice Act
• Experience operating in a skilled nursing facility (SNF) setting
• Ability to collaborate effectively with the interdisciplinary care team
• Familiarity with following physician orders and PT-directed plans of care
