Roles & Responsibilities

• Provide compassionate hospice nursing care in patients' homes, nursing homes, group homes, or assisted living facilities
• Perform thorough patient assessments and lead individualized care planning focused on comfort and quality of life
• Manage an assigned caseload as the Primary RN, coordinating and leading the interdisciplinary team for each patient
• Implement expert symptom management and clinical interventions with excellent assessment and critical thinking
• Educate patients and caregivers to build confidence in at-home care and align care with patients' personal goals
• Document clinical care accurately and communicate effectively with the team and families
• Offer occasional weekend backup and participate in on-call rotations to ensure continuous patient support

Qualifications

• Active Registered Nurse (RN) license (eligible for Colorado licensure if not currently licensed in-state)
• Experience in hospice, palliative care, or home health nursing preferred
• Strong clinical assessment skills and demonstrated symptom management expertise
• Excellent critical thinking, clinical judgment, and interpersonal communication
• Current BLS certification (additional hospice/palliative certifications a plus)
• Comfort providing care in private homes and long-term care settings; reliable transportation for home visits

Education

• Associate or Bachelor of Science in Nursing (ADN/BSN); BSN preferred
• Continuing education or certifications in hospice/palliative care encouraged
