Physical Therapist
Location: Yountville, California
Start Date: ASAP
Contract Type: Contract
Shift Type: Monday-Friday, Day shift (typically 40 hours/week; exact start/end times vary by facility)
Hours per Week: 40
Candidate Pay Rate: $66.5 per hour
Roles & Responsibilities
- Perform comprehensive patient evaluations and create individualized treatment plans that make a real difference
- Implement therapeutic interventions to improve strength, mobility, balance, and independence
- Monitor patient progress and adjust plans to ensure measurable improvements
- Educate patients and caregivers on home exercise programs and safety strategies for lasting outcomes
- Maintain accurate, timely documentation in line with facility and regulatory standards
- Collaborate with interdisciplinary team members to deliver coordinated, patient-centered care
Qualifications
- Active California Physical Therapist (PT) license
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program
- Current CPR/BLS certification
- Minimum 1 year of PT experience preferred (SNF, outpatient, or acute care experience is a plus)
- Strong communication and interpersonal skills
Education
- Doctor of Physical Therapy (DPT) preferred; Bachelor's or Master's in Physical Therapy acceptable where applicable

- Patient evaluation and functional assessment skills
- Individualized treatment planning and implementation
- Therapeutic exercise, balance and mobility training techniques
- Clinical documentation and regulatory compliance
- Interdisciplinary collaboration and patient/caregiver education
