Roles & Responsibilities

• Conduct thorough patient evaluations to identify functional goals and needs
• Develop and implement individualized treatment plans focused on meaningful progress
• Provide daily treatments that help patients regain mobility, independence, and confidence
• Manage timely and safe discharges with clear home and follow-up instructions
• Document patient progress accurately and communicate updates with the rehab team
• Collaborate with nurses, aides, and therapy colleagues to deliver coordinated care
• Maintain productivity expectations and follow CRG policies and procedures

Qualifications

• PT or DPT credential (DPT listed as a primary skill)
• 1–3 years of clinical experience (listed requirement)
• Prior SNF experience strongly preferred
• Willingness to follow facility dress code (black scrubs) and CRG policies
• Comfort working on-site (therapist must be present in the building to receive guaranteed hours)

Education

• Doctor of Physical Therapy (DPT) or equivalent PT degree preferred

Knowledge & Skills

• Skilled in patient evaluations and creating goal-driven treatment plans
• Experience providing daily treatments and managing discharges in a rehabilitation setting
• Strong documentation and communication skills to keep the interdisciplinary team aligned
• Ability to meet productivity expectations while delivering high-quality patient care
• Familiarity with SNF workflows and patient populations is a strong plus
