Roles & Responsibilities

• Evaluate residents through thorough assessments and develop individualized treatment plans
• Deliver hands-on skilled therapy focused on mobility, balance, strength, and functional independence
• Document progress clearly and timely in the medical record to support continuity of care
• Collaborate with nursing, OT, SLP, and social work to coordinate discharge plans and goals
• Educate residents and families on home programs and safe mobility strategies
• Participate in therapy team meetings and contribute to quality improvement initiatives

Qualifications

• PT or DPT credential (PT/DPT) indicated as core skills
• Any experience level welcome, including new graduates (years of experience: any, including New Grad)
• Comfort working in a skilled nursing facility (SNF) setting
• Strong communication and teamwork skills to coordinate resident care

Education

• Doctor of Physical Therapy (DPT) or equivalent physical therapist education; new grads welcome

Knowledge & Skills

• Geriatric rehabilitation principles and SNF-focused care
• Patient evaluation and skilled therapeutic intervention planning
• Functional mobility training, transfers, gait training, and fall prevention
• Clear clinical documentation and goal-oriented progress notes
• Ability to collaborate effectively with interdisciplinary teams
• Compassionate patient and family education skills
