Roles & Responsibilities

• Provide person-centered physical therapy evaluation and treatment for skilled nursing residents
• Develop and implement individualized treatment plans that improve mobility, function, and quality of life
• Document assessments, progress notes, and discharge plans accurately and on time
• Collaborate with nursing, activities, and interdisciplinary team members to coordinate care
• Educate residents and families on home exercise programs and safe mobility strategies
• Participate in care conferences and contribute clinical expertise to resident plans

Qualifications

• Licensed or license-eligible Physical Therapist in Minnesota (DPT preferred)
• Graduates and clinicians with any level of experience are encouraged to apply — New Grads welcome
• Comfortable working in a skilled nursing facility (SNF) setting
• Strong communication skills and ability to work collaboratively with an interdisciplinary team
• Current basic life support (BLS) may be required by facility (obtainable if needed)

Education

• Doctor of Physical Therapy (DPT) or equivalent physical therapy degree; new graduates welcome

Knowledge & Skills

• Skilled in assessment and treatment planning for geriatric and post-acute populations
• Knowledge of SNF documentation, Medicare/rehab goals, and transitions of care
• Proficiency with mobility training, therapeutic exercise, and functional independence strategies
• Excellent interpersonal skills for resident and family education
• Ability to manage caseload, prioritize tasks, and maintain timely documentation
