Roles & Responsibilities

• Evaluate patients’ functional abilities and develop individualized treatment plans
• Deliver hands-on therapeutic interventions and guide patients through exercises
• Monitor progress and adjust treatment plans to maximize outcomes
• Educate patients and caregivers to promote safe, independent function
• Collaborate with clinic team members to coordinate high-quality care
• Document patient care accurately and timely, ensuring continuity and compliance

Qualifications

• Current Physical Therapist license in Minnesota or eligible for Minnesota licensure
• Active clinical experience in outpatient physical therapy (clinic experience preferred)
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Strong communication skills and a patient-centered approach
• Ability to pass credentialing and background checks required by the clinic

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy
• Relevant continuing education in outpatient musculoskeletal rehabilitation encouraged

Knowledge & Skills

• Expertise in therapeutic exercise, manual therapy, and rehabilitation techniques
• Skilled in patient assessment, treatment planning, and outcome measurement
• Comfortable with hands-on healing and family-centered care
• Good documentation skills and familiarity with outpatient clinic workflows
• Team-oriented mindset and ability to collaborate with interdisciplinary staff
