Roles & Responsibilities

• Evaluate patients’ functional needs and create clear, individualized treatment plans
• Implement evidence-based therapeutic interventions to improve mobility, strength, and independence
• Monitor patient progress and adjust plans to achieve measurable outcomes
• Document care and outcomes thoroughly and timely in the facility EMR
• Collaborate with interdisciplinary team members to coordinate comprehensive patient care
• Educate patients and families on home programs and recovery expectations
• Contribute to discharge planning to ensure safe transitions and continuity of care

Qualifications

• Licensed Physical Therapist (PT) or eligible for licensure in New Hampshire
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Comfortable working day shifts across weekdays and weekends as needed
• Strong clinical assessment and manual therapy skills with excellent communication
• Able to work independently while collaborating closely with nursing and rehab teams
• Available for a 13-week contract assignment (June 1 – Aug 29, 2026)

Education

• Doctor of Physical Therapy (DPT) preferred
• Bachelor’s or Master’s in Physical Therapy or related field accepted

Knowledge & Skills

• Skilled in musculoskeletal and functional mobility assessments
• Proficient with therapeutic exercise, gait training, and assistive device training
• Knowledgeable in manual therapy techniques and therapeutic modalities
• Familiar with documentation best practices and EMR workflows
• Strong clinical reasoning and outcome-driven treatment planning
• Infection control practices and safe patient handling
• Excellent interpersonal skills for patient and family education
• Ability to collaborate within a multidisciplinary care team
