Roles & Responsibilities

• Perform comprehensive patient evaluations to inform individualized care
• Develop and implement targeted treatment plans for each patient
• Provide skilled therapy focused on mobility, balance, gait training, and improving functional independence
• Complete timely documentation including evaluations, daily notes, progress reports, and discharge summaries
• Collaborate with interdisciplinary team members such as OT, SLP, nursing, and physicians to coordinate care
• Ensure compliance with Medicare and facility guidelines
• Educate patients and caregivers on home exercise programs and safety strategies to support long-term recovery

Qualifications

• Active California Physical Therapist license
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• SNF or post-acute experience preferred
• Strong communication and documentation skills
• Ability to manage productivity expectations in a fast-paced setting

Education

• Graduate of an accredited Physical Therapy program

• Physical Therapy principles and patient evaluation

• Mobility training, balance training, and gait training techniques
• Documentation best practices and timely clinical notes
