Roles & Responsibilities

• Perform thorough physical therapy evaluations and develop individualized plans of care
• Deliver skilled treatment across settings — patient rooms, hallways, outdoor areas, and the rehab gym
• Document care accurately and timely using Cerner and NetHealth to support continuity and outcomes
• Collaborate closely with the rehab team and nursing staff to communicate progress, equipment needs, and discharge plans
• Participate in rounds and care conferences to advocate for patients and optimize interdisciplinary care
• Provide safe, effective therapy across acute, swing bed, and skilled nursing populations, adapting treatment to each setting
• Support patient experience goals by delivering compassionate, person-centered care

Qualifications

• Current NC Physical Therapy license (required)
• CPR certification (required)
• Minimum 2 years of clinical experience (job lists 2 years); preferred clinicians have 3+ years in a medical setting
• Experience working in hospital and/or nursing home settings (must have)
• Comfortable floating between acute/swing, nursing home, and outpatient services
• Familiarity with Cerner and NetHealth documentation systems is a strong plus

Knowledge & Skills

• Skilled in physical therapy evaluations, plan of care development, and progress documentation
• Knowledge of typical rehab gym equipment and treatment techniques for varied patient populations
• Ability to communicate effectively with interdisciplinary teams and participate in care conferences
• Strong organizational skills for scheduling and managing a full caseload
• Comfort with working in multiple treatment environments (gym, bedside, outdoor areas)
