Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, flexible schedule

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services to patients in their home setting
Develop and implement individualized treatment plans to improve mobility and function
Conduct assessments and document progress using Oasis and Cerner EMR
Collaborate with interdisciplinary team to coordinate patient care
Educate patients and families on therapeutic exercises and home safety

Qualifications

Valid Physical Therapy license in California
BLS certification required
Minimum 1 year of PT experience; home health experience preferred
Experience with Oasis documentation required
Must be comfortable working independently in home health settings

Knowledge & Skills

Proficiency with Cerner EMR system
Knowledge of Oasis government-mandated forms
Experience treating ortho, stroke, joint, and spinal injury patients
Strong communication and time management skills
