Contract Type: Travel Contract

Shift Type: Monday-Friday, Day shift, no weekends or holidays

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive home health assessments and develop individualized care plans
Provide skilled physical therapy interventions to orthopedic and geriatric patients
Collaborate with the interdisciplinary team to ensure coordinated care
Document patient progress in HCHB EMR system
Educate patients and families on home exercise programs and safety

Qualifications

Active Physical Therapist license in California
Current BLS certification
Home Health experience required (new travelers considered with experience)
Comfortable using HomeCare HomeBase (HCHB) EMR

Knowledge & Skills

Comprehensive knowledge of orthopedic and geriatric rehabilitation
Proficient in evaluating and treating patients with multiple comorbidities such as CHF and COPD
Ability to perform SOC visits independently
Excellent communication and time management skills
