Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes throughout Tehama County
Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
Develop personalized treatment plans for patients recovering from joint replacements and managing chronic conditions like CHF and COPD
Complete approximately 5 meaningful patient visits each day using your clinical expertise
Document care efficiently using the HomeCare HomeBase EMR system on your provided tablet
Collaborate with the intake RN and office manager to coordinate optimal patient outcomes

Qualifications

Current Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Proficiency with HomeCare HomeBase (HCHB) EMR system preferred

Knowledge & Skills

Home Health PT expertise
Experience with orthopedic and geriatric patient populations
Strong clinical assessment and care planning abilities
Comfortable working independently in home settings
Excellent communication and documentation skills
