Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy to 5-7 patients daily in the comfort of their own homes
Help patients recover from surgeries, falls, and other challenges with compassionate, hands-on care
Collaborate with Home Health and Hospice teams to support patients across their full care journey
Document progress accurately using the HomeCare/HomeBase EMR system
Travel within a 60-mile radius of Valley City to bring expert therapy directly to those who need it most

Qualifications

Current North Dakota Physical Therapy License
BLS certification
At least 1 year of physical therapy experience with previous home health background
Valid driver's license and auto insurance
Strong PT-related skills and a passion for helping patients in their home environments

Knowledge & Skills

Home health physical therapy expertise
Comfort working independently without on-site manager supervision
Familiarity with HomeCare/HomeBase EMR documentation
Ability to cover a 60-mile radius around Valley City
Experience treating post-surgical patients, fall recovery, and hospice care
