Roles & Responsibilities
- Provide compassionate social work services to patients and families
- Coordinate care plans that support patient well-being and recovery
- Conduct thorough assessments to identify patient needs and resources
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Support patients through challenging mental health situations with empathy and professionalism
Qualifications
- Active LCSW license in California
- Current BLS certification
- 5150 certification in California required
- Prior experience with Dignity Health facilities
- Strong communication and interpersonal skills
Knowledge & Skills
- Expertise in involuntary psychiatric evaluation processes
- Understanding of mental health crisis intervention
- Ability to work effectively within multidisciplinary teams
- Knowledge of community resources and support services
- Strong documentation and care coordination abilities
