Roles & Responsibilities

• Perform comprehensive assessments and coordinate individualized care plans for hospice patients
• Provide expert pain and symptom management aligned with each patient's unique plan of care
• Educate and compassionately support patients and families through end-of-life decisions and self-care techniques
• Collaborate daily with physicians, social workers, chaplains, and volunteers to deliver holistic care
• Maintain accurate, timely documentation that meets all regulatory and organizational standards
• Participate in on-call rotation to ensure 24/7 support for patients and families in need

Qualifications

• Current Registered Nurse (RN) license in Utah or compact state
• Minimum 3 years of nursing experience, preferably in hospice, home health, or palliative care
• Current BLS certification required
• Valid driver's license and reliable transportation for home visits
• Strong clinical assessment and communication skills
• Compassionate approach to supporting patients and families during difficult times
• HospiceSoft/Careficient EMR experience preferred
